The collision occurred in the seventh inning of the Red Sox's home game against the Cleveland Guardians, with Cleveland holding a 2-0 lead.

As Guardians outfielder Estevan Florial popped up to shallow left field, three Red Sox defenders rushed to catch the ball. In a miscommunication, both Devers and O'Neill seemed to call for the ball simultaneously. This confusion resulted in them colliding while looking up, with Devers backpedaling into O'Neill. The impact caused O'Neill's head to snap back, and his glasses and hat were sent flying. The force of the collision left both players tumbling to the ground.

Devers lay face down on the turf for a few moments, while O'Neill was able to take a knee to reveal a bleeding forehead. Remarkably, Devers managed to hold onto the ball, recording the third out of the inning. However, O'Neill's injuries were more severe, and he had to be replaced by Jarren Duran in left field. Ceddanne Rafaela moved to center field, and David Hamilton entered the game as the new shortstop.

O'Neill, whose hot start to the season included a batting line of .304/.448/.761 with seven home runs and eight RBIs in 14 games, had to undergo immediate medical attention. The Red Sox did not provide an immediate update on his condition, but reports suggest that he received eight stitches and entered the concussion protocol.

While Devers managed to continue playing, the collision surely left both players shaken. Despite their efforts, the Red Sox couldn't recover from the incident, and the Guardians went on to secure a 6-0 victory.
