Northwestern Medicine has successfully performed one of the first awake kidney transplants, offering hope to patients seeking faster recovery times and groundbreaking advancements in the field of transplantation. The innovative procedure, conducted by surgeons Dr. Satish Nadig, Dr. Vinayak Rohan, and anesthesiologist Dr. Vicente Garcia Tomas, allowed the patient, John Nicholas, to avoid general anesthesia and narcotics after surgery.

John Nicholas, a 28-year-old Chicago resident with Crohn's disease, required a kidney transplant due to inflammation in his kidneys preventing proper function. Initially planned to receive a kidney donation from his mother, Nicholas had to seek an alternative donor after his mother's breast cancer diagnosis. Fortunately, a childhood friend proved to be a suitable match.

The surgical team proposed the awake transplant approach for Nicholas, considering his overall good health and background in biomedical engineering. The procedure, completed in under two hours, involved numbing Nicholas from the chest down with a spinal anesthetic similar to those administered during C-sections. This alternative method eliminated the need for a ventilator to aid breathing and reduced recovery time.

Nicholas, speaking about his experience, described the surgery as "powerful" and "surreal." Within 24 hours of the procedure, he was discharged from the hospital – a day or two earlier than patients under general anesthesia. Notably, Nicholas did not require prescription narcotics after surgery, indicating the potential for a more comfortable recovery process.

Dr. Nadig expressed excitement about the patient's involvement in their own healing, noting that the awake procedure allowed them to witness their own disease's cure firsthand. This novel approach introduces the possibility of a more participatory role for patients in their recovery journey.

Moreover, this groundbreaking transplant technique opens doors for individuals with heart or lung conditions that may complicate or prevent traditional kidney transplant procedures. Northwestern Medicine aims to establish the AWAKE program (Accelerated Surgery Without General Anesthesia in Kidney Transplantation) to expand the use of this procedure, potentially benefiting a wider range of patients.

The trio of doctors behind the procedure emphasized the importance of innovation and advancing the field of transplantation. Driven by their desire to challenge the status quo, they believe there is ample room for improvement in kidney transplant practices.
