McCaffrey, who recently signed a two-year contract extension through the 2027 season, is the reigning Offensive Player of the Year in the NFL. His electrifying and dynamic play style has made him a fan favorite both in real life and in the Madden NFL gaming community. Madden NFL's senior production director, Mike Mahar, described McCaffrey as a "human highlight reel" and praised his abilities, citing that Madden players scored over 450 million touchdowns with him in the previous edition, making him the most popular running back.

The announcement marks the first time a San Francisco 49ers player has been featured on the worldwide cover of Madden. McCaffrey's exceptional performance in the previous season saw him average 5.4 rushing yards per carry, amassing 1,459 yards on the ground and scoring 14 touchdowns, leading the league in both categories. In addition, he proved to be a versatile threat by catching 67 passes for 564 yards and seven touchdowns, culminating in a total of 2,023 combined yards from scrimmage, leading the league in this category as well.

McCaffrey, a three-time Pro Bowler, joined the 49ers in October 2022 after spending his first six seasons with the Carolina Panthers. Since his arrival in San Francisco, he has continued to thrive, racking up 2,205 rushing yards, 20 touchdowns, 119 receptions, 1,028 receiving yards, and 11 scores.
