US Air Force Contractor Fatally Injured by Drone Propeller at California Airfield

In a tragic accident at Gray Butte Airfield in California, Stephanie Cosme, a 32-year-old US Air Force civilian contractor, lost her life after walking into a rotating propeller of an MQ-9A Reaper drone. The incident occurred on September 7, as stated in the recent report released by Air Force Materiel Command.

According to the investigation report, Cosme, who was employed by Sumaria Systems, LLC, was engrossed in a data-recording device and failed to maintain situational awareness. Without looking up to determine her position relative to the aircraft, she inadvertently stumbled into the moving propeller, sustaining fatal injuries.

Brigadier General Lance French, President of the Accident Investigation Board, identified two causes for the accident. First, there was inadequate instruction or training on approaching a running drone while taking data readings. Second, Cosme's excessive focus on the handheld measurement device contributed to the loss of situational awareness.

The report also highlighted two additional factors that substantially contributed to the mishap. There was a lack of communication between the contractor test team and ground support workers, and the tests conducted on September 7 were rushed due to previous delays.

Cosme, described as "a cherished daughter, sister, fiancé, mother, and friend" in her obituary, was an avid enthusiast of crafting, baking, aviation, and unconventional things. Her tragic passing has left a void in the lives of her parents, two siblings, fiancé, and stepdaughter.

As part of the investigation, Air Force officials attempted to replicate the events leading to the accident at the same location under similar lighting conditions. The re-enactment revealed that the spinning propeller was not visible from Cosme's viewpoint, making it unlikely that she heard any warnings over the loud engine noise.

This devastating incident serves as a reminder of the risks associated with working around drones and the importance of maintaining situational awareness. It also highlights the need for proper training and effective communication to prevent similar accidents in the future.

The loss of Stephanie Cosme has undoubtedly left a profound impact on her family, loved ones, and the entire Air Force community. As the investigation concludes, efforts will likely be made to implement measures that enhance safety protocols and prevent such tragedies from occurring again.
